Use Our Handy Checklist

For your convenience, we have put together a checklist of documents which are helpful in processing a mortgage loan application. You can even print out our list for a handy reference!

  1. SIGNED AND COMPLETED APPLICATION including:

    ◻ Landlord name & address - past two years (if applicable)

    ◻ Employer name & address - past two years

    ◻ Bank names, account #'s, balances

    ◻ Open loans/mortgages - name, address, & account #'s

    ◻ Monthly payments, balances

  2. OTHER INFORMATION needed:

    ◻ W2 tax form (2 years) and 4 current pay stubs

    ◻ Bank statements for all accounts - last 3 months

    ◻ Purchase & Sale Agreement (with photocopy of deposit checks)

    ◻ Self employed people must provide:

    • 2 years signed tax returns w/ schedules
    • Profit & loss statements
    • Balance sheet on the company
  3. FORMS to be obtained at interview:

    ◻ Request for copy of tax form

    ◻ Hazardous materials questionnaire

    ◻ Information release authority

    ◻ Blanket signature authorization

    ◻ Taxpayer ID information

    ◻ Initial interview worksheet

  4. DISCLOSURES which will be provided at interview:

    ◻ Mortgage lender disclosure

    ◻ Fixed rate loan disclosure

    ◻ Servicing disclosure statement

    ◻ Uniform mortgage loan cost worksheet

    ◻ Adjustable rate mortgage disclosure

  5. CONSTRUCTION LOANS - additional info needed:
